REGISTRATION PACKET - Complete the registration packet.  Page one has a listing of items needed for registration.  The residency form (PVRF-002) describes the required documentation for proof of residence. *

CHANGE OF ADDRESS - The address change form has a listing of the required documentation for proof of residence.  The form must be completed and returned with the documentation.*

Please remember that any time your child moves you must notify the school, even if the move is within the district.  An inaccurate address in our records will affect transportation and any correspondence sent throughout the district and may also affect a student's eligibility for enrollment in the district.

*If you do not rent or own your home (for example, you are residing with a parent or friend and/or you must complete sections B through D of the Residency Document in the registration packet)  In order to show proof of residence,  you will be required to complete a notarized affidavit at the time of registration or to complete an address change.   Forms must be obtained from the Central Registration Office. 

FREE AND REDUCED PRICE SCHOOL MEALS HOUSEHOLD APPLICATION - Complete this form and return it to the registration office or your child's school if you believe your household may be eligible for free or reduced lunches.
